けむり. Just けむり. I have never heard anyone use an of those others in any context ever.
けぶり is apparently the oldest version. けむり appeared in the 1300's. Later on けぶ and けむ appeared as clipped versions, and it can be heard in kabuki plays from the 1700's and 1800's.
Basically just... there's no reason to even consider using anything other than けむり.

How would you read 煙?
煙 can be read as けぶ, けぶり, けむ and けむり, but I'm not sure which one to use and in what context.
